Under general supervision of an PRN Chief Nursing Officer and RN or RN Supervisor if applicable, LPNs carry out routine patient care assignments, work in controlled patient care situations and carry out established nursing procedures. LPNs are expected to comply with their State Administrative Code and in accordance with the standard of nursing practice at all times.

1. Accept only patient care assignments which the LPN is competent to perform.
2. Provide basic nursing care as delegated.
3. Document nursing care given and patient responses as appropriate.
4. Report changes in patient condition and/or deviation from normal to Charge Nurse/Designated RN.
5. Identify legal consequences of nursing actions.
6. Identify personal strengths and limitations.
7. Seek assistance, information and/or supervision relative to nursing actions for which:
a. The LPN lacks experience or knowledge.
b. The LPN questions appropriateness of order/intervention.
c. The LPN identifies unsafe patient care practices.
8. Seek education opportunities to increase clinical competence.
9. LPNs may not care for intravenous sites or IV medications without state IV certification and unless directed by CNO or RN.


1. Current active Licensed Practical/Vocational Nurse licensed in the state of assignment
2. Must have one year of valid nursing experience, within the last two years
3. Active Telephone Number
4. Reliable Transportation
5. Two positive work references
6. Favorable criminal background check
7. A negative TB or CXR
8. Ability to read English and chart in English


Employee must be able to perform the following physical requirements of each shift worked:
1. Near vision, hearing and talking over 90%
2. Standing 67-100% including standing for more than 30 minutes at a time
3. Stooping or squatting 67-100% including 30 minutes or more at a time
4. Bending, turning or twisting 67-100% including 30 minutes or more at a time
5. Pushing and pulling up to 50 pounds for more than 5 minutes at a time.
6. Stretching 67-100% including overhead for more than 5 minutes at a time.
7. Lifting up to and overhead 50 pounds 67-100% for more than 5 minutes at a time.

8. Walking 67-100% for more than 15 minutes at a time.
9. Practice proper body mechanics to eliminate potential injury. Frequent bending and reaching, ability to move heavy objects, visual acuity, aural acuity, manual dexterity, the ability to write legibly and the ability to perform and maintain CPR certification as required by assignment.

1. Graduate of an accredited school of nursing.
2. CPR upon hire and renewal as required by facility
3. Client specific training.

Various Home Care settings. Duties may vary depending upon assignment. Nurses may be exposed to communicable diseases and may be assigned to patients in isolation. Nurses may be exposed to infectious waste including radioactive waste material from nuclear medicine. Will have working knowledge of universal precautions.

May be at high risk for occupational exposure to blood or other potentially infectious materials.

Exposed intermittently to infectious disease, radiation, chemical, and biological hazards.

Signing this agreement is stating that the employee has not been involved in an investigation regarding patient abuse, verbal or physical, and that there are no pending investigations at time of hire. If any criminal offense, investigation of abuse or investigation regarding licensure or certification occurs while employed you must notify PRN immediately. Failure to do so may result in disciplinary action up to and including termination.
