Principal Responsibilities:

Summary Statement: The Speech and Language Pathologist evaluates and treats patients in accordance with orders from a physician through use of therapeutic procedures designed to address deficits in speech, language, hearing, swallowing and cognition and improve independence, safety and quality of life. The Speech and Language Pathologist participates as a member of the interdisciplinary team providing support and information within the area of speech therapy.

• Review available patient information and evaluate the impact of this information on the assessment and treatment process
• Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures, including home/vocational and equipment needs
• Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals
• Comply with evaluation, treatment and documentation guidelines
• Record patient progress in the patient record a minimum of every ten (10) days
• Revise treatment plans, as needed, throughout the treatment process
• Documents the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format
• Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure
• Work with nursing staff, patient and families to maximize patient’s use of functional abilities
• Establish and instruct patient and family/other care giver in exercises and therapeutic procedures to be continued following discharge
• Assist in the referral process when a patient requires additional services following discharge. Maintain current knowledge of community resources
• Attend and participate in rehabilitation team conferences regarding patient progress, problems or needs
• Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency
• Attend and participate in staff meetings and other meetings as requested
• This position must continually meet the productivity objective of 75%. PRN Home Health and Therapy reserves the right to adjust each individual employee’s productivity objective independently based upon said employee’s facility responsibilities
• Act in compliance with PRN HOME HEALTH AND THERAPY Management Policies and Procedures, State, Federal and Professional Organization regulatory standards and guidelines
• Promote the programs and services of PRN HOME HEALTH AND THERAPY through formal and informal interactions with the community
• Perform other duties as assigned.
• This position is considered nonexempt under the Fair Labor Standards Act
• The duties of this position include operating on a Saturday through Friday work week

Supporting Responsibilities:

Summary Statement: It is expected that the incumbent has a genuine appreciation for the understanding of protocol, discretion and professionalism, is able to accept and assume responsibility, and has the ability to interpret and make decision in accordance with prescribed procedures and practices of the organization. In order to provide effective and efficient services it is necessary that the incumbent be self-motivated, respectful, tactful and highly organized.
• PRN Home Health and Therapy personnel will strive to provide the resident/patient with the highest quality of rehabilitation on a consistent basis.
• Diplomatic relationships will be maintained with all levels of facility and rehabilitation staff to create a strong interdisciplinary focus. Participate in facility activities whenever possible. A name tag will be worn at all times. The regulations the facility has established for their staff will be followed. Remember we are guests in their facility.
• Programs will be developed to enhance the quality of the services provided in the facility. Program development is anticipated to occupy non-productive time. If your facility has a special need, you are encouraged to develop a program. Please communicate your ideas with your supervisor.
• PRN Home Health and Therapy staff will enhance the facilities services by freely offering information for Restorative and Nursing personnel so to maintain patients newly achieved levels of function, achieved through therapy.
• In-services will be provided to educate the facility staff on how your discipline can improve the quality of care, ease the work of the facility staff and promote the recovery process of the patient.
• PRN Home Health and Therapy staff will attend various meetings in the facility in which they work. It is anticipated they will attend a weekly Medicare meeting, patient staffings and facility department head meetings (where applicable).
• Completion of all documentation, billing, and reporting are to be accurate and timely. This includes response to physician orders, prompt evaluation, daily recording of treatment and billing information, progress notes, summaries, functional maintenance programs and updates as well as monthly billing information and keeping up to date with screenings and care plans.
• Communicate with the facility regarding your schedule. Post a schedule in the facility as to your patient case load.
• Call the PRN Home Health and Therapy office immediately to report any injury, incident or other difficulty impacting you, your peers, the facility or the residents treated by you.

• Confidentiality is to be maintained regarding patient privacy issues. Follow all aspects of the Residents Bill of Rights. A copy of this Bill of Rights is included in the PRN Home Health and Therapy manual.
• Maintain the confidentiality of all verbal, written, electronic, augmentative, and non- verbal communications, including compliance with HIPAA regulations.
• The image of PRN Home Health and Therapy comes from its employees. Recognize that you are the reputation of PRN Home Health and Therapy
• Universal precautions and standards for health are carefully followed for both residents and staff protection. Attend body substance isolation in-services in your facility annually.
• Verify all PRN Home Health and Therapy required vaccinations are up to date and reported to the corporate office.
• This position is considered nonexempt under the Fair Labor Standards Act
• The duties of this position may include operating on a Monday through Sunday work week

Education Requirements:
• Graduate of an approved program in Speech/Language Pathology curriculum
• Current license/certification in Wisconsin
• American Heart Association CPR Certified
• Recent graduates are welcome
Training, Skills, Knowledge and/or Experience:
• Certification by the American Speech Language/Hearing Association
• CFY supervision available
• Excellent oral and written communication skills
• Excellent interpersonal and problem solving skills
• Knowledge of accreditation standards and compliance requirements
• Ability to work an 8 hour shift and to walk, bend, drive, stand, sit and reach constantly during that shift
• Ability to push wheeled equipment throughout the facility
• Ability to participate in sustained activity for up to 3 ½ hours at a time
